So there are two main types of snapping hip syndrome.
The first type is external, meaning this happens outside of the joint. Oftentimes it's the IT band which is a thick fibrous tissue along the outside of your joint that moves over a bony protuberance of the femur. This type of snapping hip syndrome is oftentimes painless, and people may even just notice noise without any pain or discomfort. Sometimes patients describe it as being like their hip dislocates, but I assure you that nothing is falling out of place or dislocating with it. If this noise is bothersome, though, or does start to cause some discomfort to the outside of your hip, physical therapy can be really useful to help strengthen the glute muscles, improve your joint range of motion, and stabilize things within your pelvis floor so that doesn't happen anymore.
The other type of snapping hip syndrome is internal, meaning it's happening very close to the joint line or within the joint itself. One type is illiopsoas tendonitis, which is a tendonitis or inflammation of the hip flexor tendon. This can cause discomfort and a loud snapping noise when the tendon moves over the hip joint, and can be associated with sharp pain. Physical therapy can be useful for this to help improve the strength and flexibility of that tendon, as well as strengthening the other muscles around the hip joint. In cases where physical therapy is not helping, an ultrasound guided coritsone injection can be used to help relieve inflammation.
The other type of internal snapping hip syndrome can come from a labral tear. Physical therapy can also be useful to help improve joint range of motion, improve strength around the joint and help maximize joint function. But in cases where physical therapy isn't working, you may need an MRI to help diagnose or even an ultrasound guided injection for diagnostic purposes. In extreme cases, for internal snapping hip syndrome surgery might be performed such as iliopsoas tendon release or as a labral repair arthroscopic surgery.
